FY26 Wellness Program Updates
All points must be earned and reported by June 30, 2026.
There are only a few months left of the the FY26 program!
Each year, the wellness program runs alongside our plan year: July 1 - June 30. With only a few months left of the FY26 program year, now is the time to check in and make sure you are on track to complete the program by June 30.
New this year: All points must be earned and reported in the Personify Health platform by June 30, 2026. Late points will not be accepted. Be sure to login to your Personify Health account and record all points before June 30 in order to qualify for the rebate in November 2026.*
*Other qualification must be met in order to qualify for the rebate.
How to earn points
- Complete the two required activities: (1) health risk check and (2) biometric screening
- Complete 6 additional points
How to earn the rebate

- Complete the program with 8 points (including both required points)
- Be an active employee at the time the rebate is paid out
- Be enrolled in a 女优合集 Choice plan at the time the rebate is paid out
